Our Housing Support Services are designed to support individuals in finding, securing, and maintaining stable, safe, and affordable housing. Through a person-centered approach, we provide guidance and hands-on assistance every step of the way—from the initial housing search to long-term tenancy support.

Housing Information & Assistance

We help participants understand and navigate various housing programs by providing:

  • Clear explanations of housing program rules and eligibility requirements
  • Assistance with housing searches and applications
  • Support in gathering necessary documents, such as state ID, birth certificate, Social Security card, and proof of income or benefits
  • Assessments to identify preferred and safe living environments ready for move-in
  • Help requesting reasonable accommodations under the Fair Housing Act
  • Guidance on finding resources for security deposits, moving costs, furniture, utilities, assistive technology, and other one-time expenses
  • Review of leases and property rules before signing
  • Budget planning and financial education
  • Strategies for addressing housing barriers such as poor credit, rental history, criminal background, or behavioral challenges
  • Support in resolving housing-related disputes

Housing Transition Services

These services help individuals prepare to move into housing successfully by offering:

  • Tenant screening and housing assessments to identify potential barriers and retention risks
  • Development of an individualized Housing Support Plan that outlines:
    • Short- and long-term housing goals
    • Strategies to overcome barriers
    • Natural supports, service providers, and community resources to assist in achieving goals

Housing Tenancy Sustaining Services

Once housed, we continue supporting participants to maintain their tenancy through:

  • Education on tenant and landlord rights and responsibilities
  • Coaching on how to be a respectful and responsible tenant
  • Support with lease compliance and recertification processes
  • Assistance in building positive relationships with landlords and neighbors
  • Early intervention when behaviors threaten housing stability
  • Mediation and dispute resolution with landlords or neighbors
  • Advocacy and connection to resources to prevent eviction
  • Ongoing reviews and updates to the Housing Support Plan to reflect changing needs
